What pointers to remember in order to keep our web servers fully optimized and running smoothly and what are the things that can bring it down if not taken care off?

Letting your hosts go crazy with storage and cause problems, not watching your load balances, letting users run their own scripts which can bog down the server. Basically watch everything about it. Stay up on security issues and you should be ok.

Hi,
What are the files we have to look at it?, I normally use top command, but only
we can find the percentage.
How to prevent it?

Just a suggestion, Streaming can be a heavy load on your server. Try to avoid it.
So is IRC. We can't stress this enough, IRC can kill your server =P.

Again why I say, closely monitor your server. Do your best a moving accounts around to keep a low server balance. When a customer starts to complain follow up on the problem and if possible locate the offending customer. You are going to have several spikes every once and a while.

Just watch for your loads. and monitor your server frequently.
