I can't really give you steps, but a good server admin is a must, also if you include server updates to your down time, don't, just tell your customers that you are expecting downtime and tell them for how long."Programming today is a race between software engineers striving to build bigger and better idiot-proof programs, and the Universe trying to produce bigger and better idiots. So far, the Universe is winning."

The biggest problem with downtime is hackers. Keep your programs UPDATED at all times (the kernel is almost never the problem), the update for programs never needs a reboot so you don't have to deal with downtime. If you need to do downtime, do like jimbob said tell people why its going for a reboot what date and time and give a good reasonable amount of time so people can be notified (a week is what i give my clients)

as far as your server has been setup correctly and had the right security programs and firewall installed and you keep an eye on things and keep your cpanel and whm or whatever control panel you use, you should have no problems...i have had no downtime in four months except for the kernal updates that have been released due to security loopholes that have been found...downtime should just not happen if your server is located in a great datacenter
